If I buy Throne of Destiny from Direct2Drive, does it come with the activation key?

Yeah it is supposed to. That is the whole point of buying it. You can get the software for free by just downloading it from Turbine's web site.

One more thing, if you don't mind... My account now says "Throne of Destiny Standard" with an upgrade button below it. Is there a higher level than "Standard?"

There are three kinds of accounts. One of which was a limited time offer that ended in January 2006.
1) Dark Majesty
2) Throne of Destiny
3) Throne of Destiny with preorder bonus --> no longer available.

Turbine continually changes (once a week) the way their billing system works and how it displays information to the users. As to what it supposed to look like today - No Idea. Next update is Monday, so it may be different in the afternoon after the update.
